Important: You'll need a medical form for students, so here's a generic medical release that you may use.
Please complete carefully - first fill in the church name (3 places), then photocopy for each student. Don't send these with your registration, but bring them to camp. Hint: check each camper's for parent's signature before the student gets into the vehicle!
